Trainers provide a mount for a normal bicycle to essentially turn it into an indoor exercise bike. The small roller shown in the image sits against the rear tire and provides resistance so that it feels more like riding outside.
Some trainers simply attach to the back of the bike, clamping onto the rear wheel and holding the bike upright. Other trainers hold the back wheel, too, but require the removal of the front wheel so the fork can be clamped onto a front holder. The style that only holds the rear of the bike is the most popular because they take up the least room so can be stored and transported easily. More expensive versions come with speedometers, power meters, or can be hooked up to a computer with software to race against others.

It turns your bike into a stationary exercise bike. You want a smooth tire on the rear to keep the noise and vibration at a minimum.
